How to claim Allianz Medical and Health Insurance?

This is the process for medical claim

  • Submit the complete document to Allianz life branch
  • Allianz receives document and processes claim
  • Claim approved (7 working days)
  • Claim payout via E payment (for living claim) or cheque (for death claim) (5 working days)

  • Original Receipts
  • Original Final Tax Invoices
  • Itemized Bills / Detailed Bills indicating:

    – Test/Investigation Charges
  • Diagnostic Reports, for example:
    – Magnetic Resonance Imaging (MRI)
    – X-ray
    – Histopathology Examination (HPE)
    – Blood Test
    – Computed Tomography (CT) Scan
    – Coronary Angiogram
    – Electrocardiography (ECG)
  • For Overseas Treatment (not required for Singapore and Brunei) please include:
    Proof of Life Assured leaving Malaysia:
    – Life Assured’s Passport Copy indicating date of departure from Malaysia
    – Boarding Pass
    – Travel Ticket Details
  • Following supporting documents in English:
    – Medical Report

How to pay Allianz Medical and Health Insurance?

There are five sources of mode of payments

1. Online payment via MyAllianz customer portal

You can pay for your policies conveniently on the MyAllianz Customer Portal via the following:

  • Debit / credit card – payment will be deducted from your card on a recurring basis when your premium is due
  • Online Banking (FPX) – payment will be deducted from your current / savings account on a one-time basis

2. JomPAY

A payment solution that allows you to make online payment anytime and anywhere via your saving or current account or credit card. You can make either a onetime payment or opt for an automatic recurring payment.

3 easy ways to start JomPAY:

Step 1: Look out for the JomPAY logo and Biller Code on your bill

Step 2: Log on to your preferred internet or mobile Internet Banking and look out for the JomPAY icon

Step 3: Enter the Allianz Biller Code 5017 followed by your Allianz Life Policy Number and premium amount to make your payment. One policy per transaction.

 3. MYClear Direct Debit services

You can make your premium payment using any of these MYClear Direct Debit participating banks:

You can walk in to any of our nearest branch to pay for your premium via the following methods:

  • Cash
  • Cheque – Cheques should be crossed to “Allianz Life Insurance Malaysia Berhad”. Maximum 8 policies are allowed in one cheque.
  • Credit Card – Only Visa and Master Card are acceptable.

5. Recurring auto pay with debit/credit card

  • You can arrange to have your debit / credit card debited automatically whenever your premium is due on a recurring basis via the MyAllianz Customer Portal.
  • Login to MyAllianz Customer Portal.
